Our Village at St. Clair , located in Windsor, Ontario has an exciting opportunity for a RESIDENT SUPPORT COORDINATOR (externally known as a Social Worker or a Social Service Worker) to join their village team.

As our new Resident Support Coordinator, you will:
- Have a degree in Social Work or related field such as Social Service Worker with a specialty in gerontology
- Be registered with the Ontario College of Social Workers & Social Service Workers
- Coordinate and support the transition of residents moving into a health care setting
- Support resident and family meetings, care conferences and therapeutic groups
- Provide education, training, coaching and counselling for residents, families and team members regarding mental health and well-being
- Support residents on the waitlist and ongoing assessment needs with residents and those waiting for services
It would be an asset if our new Resident Support Coordinator has:
- Current Heart Save Level of CPR
- Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, and Health & Safety Act of Ontario
- Demonstrated experience working with seniors and/or persons with cognitive or physical impairments
